AFFIDAVIT OF LOSS

 

I, XXX, of legal age, Filipino, widow, with residence address at _________________________, after having been sworn in accordance with law, hereby deposes and states the following, THAT:

 

1.      I am the wife of deceased XXX who operated a printing services business with Business/Trade Name of XXX located at ____________________;

2.      That XXX was duly registered with Bureau of Internal Revenue (BIR) with Tax Identification No. XXX and was therefore issued a BIR Certificate of Registration (COR);

3.      Consequently, he was issued with Authority to Print (ATP), Notice to Issue Receipt/Invoice (NIRI) and Accreditation Certificate and Permit to use;

4.      After his death, all these afore-mentioned documents including the unused receipts, were under my possession;

5.      However, despite diligent efforts to search the COR, ATP, Accreditation Certificate and the unused receipts, they could not be found and the same are now beyond discovery and only NIRI was found;

6.      As such, I am executing this affidavit to attest to the truth of the foregoing as well as to support my request for closure of business and for whatever legal purpose it may serve.
