          COMES NOW, the undersigned Petitioner, unto this Honorable Court, most respectfully avers the following:

 

1.     That the petitioner is Filipino, having been born in the Philippines on __________ and currently residing at _______________________________. Copy of Petitioner’s Birth Certificate is hereto attached as Annex “A”;

 

2.     That the Petitioner has been admitted to the Philippine Bar on ____________ with Roll No. ________, after passing the bar examination given on _________________. Copy of the Certificate of Membership is hereto attached as Annex “B”;

 

3.     That the Petitioner has no pending administrative case against her as certified by the Office of the Bar Confidant of the Supreme Court of the Philippines on ___________.  Copy of the Certificate of No Pending Case is hereto attached as Annex “C”;

 

4.     That the Petitioner has neither been convicted before any court or quasi-judicial body of a crime involving moral turpitude nor has been charged of any such offense. Petitioner has acquired a Clearance dated ____________ and valid until ___________, issued by the National Bureau of Investigation (NBI). Copy of the NBI Clearance is hereto attached as Annex “D”;

 

5.     That the Petitioner is a member in good standing of the Integrated Bar of the Philippines (IBP) with ____________. Copy of the Certificate of Good Standing, IBP ID, Official Receipts for the payment of IBP dues and Professional Tax as Annex “E” “F” “G” “H”, respectively;

 

6.     That Petitioner has not yet complied with the required units of the Mandatory Continuing Legal Education (MCLE) since she just been admitted to the Philippine Bar on _____________ but will be seeking an MCLE schedule to enroll with anytime and comply with the units mandated by the Supreme Court; [If with compliance, provide the Certificate of Compliance]

 

7.     That the Petitioner is currently holding an _______ position at (Government Agency) but she was able to secure permission or authority to engage in the limited Practice of Profession from __________ signed by the _________________on ________________. Copy of the Permission or Authority to Engage in Limited Practice of Profession is hereto attached as Annex “I”; [Applicable only to lawyers employed by the Government]

 

8.     That by virtue of said Permission or Authority issued in favor of the Petitioner, she has established her office at ________________ , with number ____________;

 

9.     In compliance with other requirements for the grant of a notarial commission also attached are the following:

a. Three specimen of her official signature over printed name, as Annex “J”

b. Three pieces of Passport-sized pictures with signature affixed, as Annex “K”

                   c. Picture of office together with the Petitioner, as Annex “L”

                   d. Three imprints of dry seal, Annex “M”.

 

PRAYER

WHEREFORE, Petitioner most respectfully prays of this Honorable Court that after due notice, publication and hearing, a Commission as a Notary Public for and within this jurisdiction of this Honorable Court be issued in favor of herein Petitioner.

Other reliefs just and equitable under the circumstances are likewise prayed for.
